Attire: Workout Attire and Cleats.
PLEASE DO NOT BRING ANY OTHER FOOTBALL EQUIPMENT OR PADS.
Required Forms for Try-Out
Please Bring:
-Tryout Waiver Completed and Printed
-Sickle Cell Screening and Informed Consent Form Completed and Printed
-Utah State Student ID Card
-Copy of Unofficial High School Transcript
-Proof of Physical completed by an MD or DO within the last 6 months (can be completed through campus health)
-Proof of Medical Insurance
